A legal disclaimer
I, the undersigned, confirm that I am the parent or legal guardian of the child named above and hereby give permission for them to participate in a children’s Pamper Party hosted inside The Pamper Van, operated by STEEL CITY PAMPER VAN
I understand that the event includes a variety of pampering activities such as (but not limited to): mini manicures, foot spa pedicures, spa facials, hair styling, face and hair glitter, and other hair and beauty based treatments/services.
Waiver of Liability
I acknowledge and accept that although every reasonable effort is made to ensure the safety and well being of all children, participation in any activity carries inherent risks. By signing this form, I voluntarily assume all risks associated with my child’s participation and agree to release, waive, and hold harmless STEEL CITY PAMPER VAN, its directors, employees, contractors, and volunteers from any and all liability for injury, loss, or damage, including personal or property loss, that may arise during or as a result of the event.
Health & Medical Consent
- I confirm that my child is in good health and physically able to participate in the planned activities.
- I consent to basic first aid being administered to my child if needed.
- In the event of an emergency, I authorise STEEL CITY PAMPER staff to seek appropriate medical treatment for my child if I am unable to be contacted.
- I understand that any medical expenses incurred will be my responsibility.
Allergy & Sensitivity Disclaimer
I understand that steel city pamper van uses a range of beauty products and treatments that may contain common allergens or skin irritants. I agree to disclose any known allergies or sensitivities related to my child prior to the party and will notify staff of any updates. While reasonable precautions are taken, I acknowledge that steel city pamper van cannot guarantee a completely allergen free environment, and I agree not to hold the business or its staff liable for any allergic reactions or adverse effects.
Photo & Video Consent
I grant permission for Steel city Pamper van to photograph and/or record video footage of my child during the event. These images may be used for promotional purposes including, but not limited to, social media, websites, print materials, and advertising. I understand that no financial compensation will be provided for the use of these images. If you do not wish for your child to be photographed or filmed, please notify a member of staff in writing prior to the party.
Acknowledgement & Agreement By Confirming on booking that I confirm that:
- I have read and fully understood this waiver and terms of participation.
- I have had the opportunity to ask questions regarding the activities and policies.
- I agree to abide by the rules and expectations set by STEEL CITY PAMPER VAN.
- I understand this waiver applies to this and all future events my child attends with STEEL CITY PAMPER VAN unless otherwise stated in writing.
